By referring a young person to this new program, you are offering them the one-on-one support of a case manager - a trained youth caseworker, who will support them to make and attend appointments with the specialist youth services that best suit their needs.
The Change Your Course Program (The Program) is funded by the Federal Government and administered by Streetwork Australia as the lead agency.
The Change Your Course Program is available to vulnerable young people - 12-24 - living in the Northern Sydney Region and parts of South Western Sydney. This free early intervention and prevention program is for youth affected by police, court orders, or the juvenile or criminal justice systems.
The program is a collaborative initiative between Streetwork, The Northern Centre, The Burdekin Association, Phoenix House, KYDS Youth Development Service, Payce Foundation, Catholic Care Diocese, Mentoring Men, Youth Up Front, and CCNB.

The Program's proactive case management and mentoring model, places the young person at the centre of a bespoke case plan designed to address their unique situation.
Goal plans
The Program empowers at-risk young people to self-determine the skills and supports they need to challenge and change their trajectory to a more positive set of outcomes.
Skill building
The Program enhances resilience, builds skill levels and develops protective factors while promoting social and emotional wellbeing.
Access to youth services
The Program combines a community network of specialist youth service partners, delivering a holistic model that reflects each young person's social and welfare needs.
